Allegations: While at Chatsworth at PGA National, the staff and employees failed to develop an adequate care plan and properly monitor the Resident in order to prevent her from suffering falls and unexplained injuries. On December 24, 2016, the Resident suffered a fall resulting in elbow trauma which required surgical repair.
RESULT: Case is on-going.
Allegations: On January 5, 2011, the Resident was at Chatsworth at PGA National when she was agitated and placed at a desk in a wheelchair. Unrestrained, and without supervision, the Resident attempted to get up and fell sustain a broken hip. On January 7, 2011, the Resident underwent a left bipolar hip arthroplasty at Palm Beach Gardens Medical Center performed by Dr. Steven R. Saslow.
RESULT: Case was disposed by Settlement on April 3, 2013.
Allegations: On February 24, 2010, Chatsworth at PGA National had an ongoing problem of water accumulating on the floor in the kitchen. An individual slipped and fell due to water that accumulated in the ground causing bodily injury.
RESULT: Case was disposed on June 4, 2014.
Allegations: During the Residents admission at Chatsworth at PGA National, the Resident developed a large, infected, painful ulcer on her right leg.
RESULT: Case was disposed on May 21, 2013.
