Grace at State Street concentrates on offering independent living and assisted living care for our residents in cozy, comfortable atmosphere.

Our community has roomy, well designed units that have several options for our senior residents. We provide several services that provide our seniors with a comfortable and secure atmosphere. Our staff works hard to maintain our principles and preserve the dignity of our senior residents from Boise. We encourage independence and celebrate the independence of our residents. Grace at State Street meal plans that are prepared with fresh ingredients daily from specially designed menus. We also offer snacks between meal times. Private dining, family style, cookouts, picnics are all available for every style of meal that could be desired. We can accommodate for nearly every type of diet.

During the lockdown of the community due to the pandemic, there are not big portion sizes of food and is served in to go containers. During the week they have more than enough staff. On the weekends they have limited staff members. If the staffing could be better, the ratings could be improved on.

The facility is nice, clean and the staff are very friendly. There are some activities which is enjoyable. The facility van is limited.The meals are the downside. Many residents complain about the quality of the meals and/or the variety .

Section : Activities Communal Dining And Outside Providers

  • Group activities will be allowed, ONLY IF social distancing measures are feasible and in place.
  • Communal dining will be allowed ONLY IF social distancing measures are feasible and in place.
  • Foot/Eye Clinics/beautician will be allowed, ONLY IF 1. Deemed essential by administrator/nursing and 2. Guests are competent in handwashing, sanitation, and disinfecting equipment between residents, and 3. They are wearing appropriate PPE.4. Care provided to one resident at a time.5. Facility nurse will identify those individuals who require the service prior to visit.6. A designated area will be specified and only 1 resident at a time will be serviced at once.
  • Home Health, Hospice, and other essential providers should don a surgical mask, or a KN95/N95 respirator when delivering cares.

    Section : Suspected And Confirmed Cases

  • If you have a resident you suspect has been infected, they will immediately be placed into quarantine and be treated as a positive case. Contact Regional Nurse for PCR testing while coordinating and obtaining a physician order for the NP swab collection. Testing will be conducted through Cole Diagnostics unless otherwise specified.
  • Regional Nurse will communicate with Public Health in the event of a positive/confirmed case.
  • Cohorting Strategy:
  • Resident with signs and symptoms will be sent to the hospital for evaluation
  • .If staffing cannot accommodate a COVID 19 resident, the resident will be sent to a COVID unit.
  • A designated staff member will be assigned to the COVID 19 resident if the resident is retained.

    • If unable to care for the resident in their current setting, they will be transferred to the appropriate acute care setting. Transport and acute care facility will be notified prior to transfer.

    Grace State Street Assisted Living offers Independent Living and Assisted Living at its senior living location peacefully set back off State Street. The developing area offers new little neighborhoods, small businesses, and nearby professional and medical offices. Our residents, like those in most senior living communities, appreciate a quiet setting close to things they and their families like to do.

    Beautiful fireplaces surrounded by sofas and chairs make a friendly or family visit comfortable for small or large groups. A fireplace and comfortable furnishings make the library area a favorite place for the card group or to just relax by the fire. Our private dining room is perfect for special occasions and family birthday parties. The piano in the dining area is often used and appreciated by the residents and by the musicians who come often to provide entertainment.

    Residents enjoy covered parking, private patios, and plenty of sidewalks for those who like a fresh air walk. All residents enjoy the movie theater with big lounge chairs and a popcorn machine. The game tables and activity rooms are popular, and our gym and salon keep everyone looking and feeling their best.

    Support as we age is a familys number one concern. It takes a lot off our residents shoulders and when they know a little help is just a button push away someone is always there.

  • PPE preservation methods remain in place. Examples include: gowns to be re-used unless wet, soiled .- If caring for a resident in isolation adhere to the following options: If utilizing re-usable gowns they will be laundered after each shift. If utilizing disposable gowns each staff member will have their own gown. Will be used for multiple shifts and stored in a paper bag just inside the residents room . If using cloth facemasks, be sure to launder them frequently.
  • All PPE requests should go through your Regional Nurse.If unable to obtain PPE through local channels, corporate purchasing will supply adequate amount of PPE.

  • Provide safe ways for residents to continue to be active, such as personnel walking with individual residents around the unit or outside.
  • Limit the number of residents or space residents at least 6 feet apart as much as feasible when in a common area, and gently redirect residents who are ambulatory and are in close proximity to other residents or personnel.
