My duties include managing all aspects of the Assisted Living/Memory Care community. This includes all scheduling, managing staff, disciplinary action and training and educating associates on all aspects of dementia care. I also run a Alzheimer’s support group twice a month. As Assistant Administrator over the entire community, I also am responsible for managing shared departments on the Transitional Rehabilitation side. In the absence of the Executive Director, I manage the entire community.

Medicaid Waivers In Salt Lake City
Aging Waiver
The Aging Waiver is a Medicaid Home and Community-Based Services Waiver that is designed to provide services to help adults aged 65 and over remain in their homes or a community-based setting. This program supports independent living through supports like adult day healthcare, meal delivery personal attendant services and respite care. Program participants are able to self-direct their care and hire the caregiver of their choice, including some family members.
To be eligible for the Aging Waiver, applicants must be 65 years old or over and require nursing home-level care. They must also meet Medicaids enrollment requirements. For a single applicant, the maximum monthly income allowance is 1,041 and the asset limit is $2,000. For couples, the maximum monthly income allowance is $1,372 and the asset limit is $3,000.

New Choice Waiver
The New Choices Waiver program is for individuals who are currently residing in a long-term nursing facility or licensed assisted living facility and wish to transfer into an integrated community-based setting if their needs can be safely met in the setting that theyve chosen. I was very upset by the treatment my grandfather got from cottonwood place. They are not equipped for memory care and when i dropped in for a visit I found him covered in feces and urine in his bed. I asked the nurse how long it had been since he had been checked on and she told me six hours. who knows how long he had to sit in that. they are rude and under staffed. i would never trust them with someone I love.

As the Executive Director of Cottonwood Place, this is the first I have been made aware of your experience. When residents and family members have an issue, they typically approach me with the problem so it can be addressed immediately. We are a fully equipped, state licensed memory care community, designed and staffed in accordance for the care of residents with dementia and Alzheimers. It is possible at times that family members may visit their loved one right after an incontinence accident occurs, but due to our communitys strict policy of hourly status checks on each resident, it is highly unlikely that six hours would have passed. Our entire care staff are devoted to each and every resident, as evidenced by our full community and low turnover rate. If you wish to further discuss the matter, please contact me.

Home and Community-Based Alternatives Program
The Home and Community-Based Alternatives program allows seniors to receive financial assistance with in-home care, avoiding or delaying nursing home placement. This state-funded program prevents seniors from having to spend down their income and enroll in Medicaid. Some services covered by this program include case management, homemaker services, skilled healthcare, respite care and medical equipment rental or purchase.
